Aftercare Program

Aftercare was designed to ensure clients and their families/guardians have the support needed to transition back home and remain well in the community when intensive services end at Youthdale. This brief three month service is available to youth and their families transitioning from Live in Treatment services, ASU and TAU services as well as our Day Treatment program.

The program offers service navigation, revisiting strategies while transitioning back to the community and assistance in facilitating warm transfers. The program also includes a follow up call after a month’s time of discharge from aftercare services to check in and determine if other supports are needed. If youth and their caregivers are not sure about accessing the additional support, there is a three month window from discharge to still receive aftercare support.


The Aftercare Service is a team of staff who will offer community based support to those transitioning from Youthdale services. You and your and family/caregiver will meet with an assigned Aftercare staff person prior to leaving Youthdale, to work on a transition plan that addresses some of the following:

·      Goal setting, therapeutic support and skill building to connect or re-connect to school, employment, social/recreation activities as well as referral to follow up addiction and mental health services.

·      Support and advocacy to address barriers to service navigation and transition to the right service(s) and support(s).

·      Safety planning and support including connection to crisis services as needed.
